This award recognizes quality improvement projects designed to improve safety and/or quality in practice. Up to two recipients will be selected each year. All are eligible to apply.

Application requirements

The application for this award includes a short form describing the patient safety or quality improvement project that was implemented. A two-page abstract is required.

The two-page abstract should include:

1. Project participant name(s)
2. Project title
3. Objective(s)
4. Background
5. Methods
6. Data and results
7. Conclusions
