Measure purpose: Ensure patients with Parkinson's disease (PD) are provided appropriate rehabilitative therapy options yearly.
The numerator: Patient who were referred to physical, occupational, speech, or recreational therapy once during the measurement period
is divided by
The denominator: All patients with a diagnosis of PD
Exclusions are removed from denominator:
- Patient and/or care partner decline referral
- Clinical determines patient does not require referral
- Patient already receiving physical/occupational/speech/recreational therapy during the measurement period
