Exercise or physical activity counseling quality measure

Measure purpose: Ensure patients with Parkinson's disease (PD) are counseled on importance of and provided regular exercise regimen recommendations yearly.
The numerator: Patients counseled on an exercise or physical activity regimen once during the measurement period
*Physical activity may include tai chi, dancing, boxing, yoga, and other non-traditional aerobic or strength training exercises

The denominator: All patients with a diagnosis of PD
Exclusions are removed from the denominator:
- Patient and/or care partner declines counseling
- Co-morbid condition that deems the patient unfit to participate in physical activity
- On date of encounter, patient is not able to participate in counseling, including non-verbal patients, delirious, severely aphasic, severely developmentally delayed, severe visual or hearing impairment and for those patients, no knowledgeable informant available.
