To evaluate the relationship between functional and ataxia severity scales in order to identify cutoff landmarks for disability and estimate the mean SARA score for the risk for falls on the Berg balance scale.

SpinocerebellarAtaxias (SCAs) are a group of neurodegenerative diseases characterized by deterioration of balance and functionality that tends to follow disease progression. Despite this seemingly clear and intuitive correlation, there is no established link between the formal clinical markers of severity and functional / balance scores.

Consecutive patients witha moleculardiagnosis of SCA (total 126 – 31 with SCA2, 58 with SCA3 and 37 with SCA10) were assessed for functionality (FIM-ADL and Lawton-IADL scales), balance (Berg balance scale) and disease severity (SARA).

The main disability cutoff landmarks in the SARA scale were: need for supervision starts from 10 points, dependency for all instrumental activities (Lawton) at 15 points in  SARA, and with 20 points, patients were dependent for all basic activities (FIM). The items with the greatest impact on the discrimination between dependents and independents were: “Food preparation” and “Responsibility for own medication” at Lawton scale and the domains for “Self-care” and “Locomotion” at FIM. The mean SARA score for patients classified as low risk in BERG was 9.4 points, 17 points for medium risk and 25.5 points for high risk.

The analysis of the correlation between the severity of ataxia and functional scales can be an important guide to understand the progression of functional dependence in individuals with SCAs and to create strategies to minimize the impact of the disease.
