Improve Neurology and Internal Medicine Resident work-life balance and burnout through ACGME approved block continuity clinic curriculum.
Residents are especially prone to burnout, with up to 75% physicians in training experiencing burnout. Consequences of burnout to residents can have a negative impact on patient outcomes and physician patient interactions. This study focused on monitoring resident responses to changes in their continuity clinic schedule by making changing from ½ day per week to a block curriculum. By implementing pre and post surveys, the project aim was to look at whether resident wellness improved through this curriculum change.
ACGME accredited Neurology and Internal Medicine residency program at Geisinger Medical Center in Danville, PA instituted a block curriculum for their continuity clinic. The schedule consisted of four to six weeks of inpatient or elective rotations followed by one to two weeks of continuity clinic. This differs from the traditional continuity clinic schedule of one-half day of continuity clinic every week for the whole year. Surveys were conducted both before and after the curriculum change asking about resident’s burnout levels, unanticipated days off taken, overall work life balance etc.
There were a total of 91 responses (56 pre survey and 35 post survey).  Residents reported a significant difference in perception of work life balance between the block curriculum and the previous longitudinal curriculum model (20% vs 3.6%, p 0.0213). This data also showed that residents who reported feeling burnout had significantly more unanticipated days off as compared to those residents who did not report burnout (89.7% vs 50%, p 0.0489).
Scheduling resident’s continuity clinic in a block curriculum style improved resident perception of work-life balance and decreased feelings of burnout. The Neurology RRC decision to allow flexibility in continuity clinic schedules has positively impacted the wellbeing of our residents at Geisinger Medical Center.
