Combating Resident and Fellow Obesity Through Intensive Meal Plan Coaching: A GMEC wellness subcommittee quality improvement pilot project.

The Geisinger GMEC wellness subcommittee’s objective was to implement a robust meal plan challenge to improve weight, BMI, and percent body fat among residents and fellows. 
Physicians in training are particularly vulnerable to barriers in wellness due to prolonged duty hours, stressful work environments, financial stressors, and disruption of work life balance. Burnout rates are staggering with up to 75% of physicians-in-training experiencing burnout.  There is a paucity of literature however on resident and fellow obesity and eating habits. 

A 4-week meal plan challenge was designed that included a strict food guide with portion specifics, weekly personalized coaching, and an optional exercise regimen.  Each resident or fellow was required to weigh in weekly to calculate BMI and percent body fat over the course of the challenge.  Self-analyzed levels of compliance and determination were also collected each week during coaching sessions.

Six residents completed the meal plan challenge.  At the start, the average weight of all participants was 189.1lbs.  3 residents were obese (avg BMI 32.2), 1 resident was over-weight (BMI 28.8), and 2 residents were normal (avg BMI 23.3).  The average percent body fat of all participants was 38.4%.   The overall change for this cohort was a loss of 42.8lbs, loss of 13.7 % body fat, and a decrease of 6 points for BMI.  5/6 residents enrolled had improvement in their baseline metrics.    Resident self-rated compliance average was 7.1 out of 10, and average determination was 8.3/10. 
An intensive meal plan challenge with individualized coaching is beneficial and effective for residents and fellows resulting in weight loss, fat loss, and improved BMI.  GME programs across the country may want to consider initiates similar to this as a resource to improve the health and wellbeing of learners.
