Wellness Initiative in Neurology Residents, Fellows and Advanced Practice Clinicians

To improve the wellbeing of neurology trainees and advanced practice clinicians (APCs), and assess the effectiveness of interventions.

Physicians experience a significant amount of burnout. AMA studies showed neurologists are among the top in physician burnout at 48%. A survey amongst our neurology residents revealed burnout and signs of depression. These findings are alarming and further supports that wellness among trainees and APCs needs to be a priority.

Surveys are sent to Neurology residents, fellows and APCs to establish baseline wellness and burnout levels, and at 6-month intervals to evaluate the effectiveness of the wellness program.  Wellness activities are tailored towards people’s interests, and were focused on physical wellbeing, community-building, and peer-support. Examples include hiking trips, potlucks, pet therapy, and resident-to-resident mentorship. Activities targeted multiple wellness dimensions, including physical, emotional, social, and environmental.

Initial survey in January 2019 showed that 90% of participants felt that wellness was important. But 80% felt that they were suffering from burnout.  Only 40% felt that they had enough time for personal relationships.  Almost half of respondents slept only 5-6 hours and ate 2 meals per day.  About 30% of respondents exercised less than 1 hour per week. 

Follow up survey in May 2019 showed some improvement. Fewer people suffered from burnout (88% vs 29%). More people felt a sense of accomplishment from their work (75% vs 57%). People were more aware of resources available to them (100% vs 43%). Half felt that they had enough time for personal relationships. More participants slept more than 6 hours per night, and 63% ate 3 meals per day.

Since project initiation, residents and fellows have provided positive feedback to the activities, indicating improved wellbeing post-intervention.  Follow up surveys showed overall improved wellness, personal fulfillment, and awareness of available resources.
