Neurological Findings In The Most Prevalent Hereditary Ataxias In Southern Brazil

To examine vestibular disorders in patients with recessive and dominat
cerebellar ataxia.
Spinocerebellar ataxia is a disease that belongs to a heterogeneous group
of neurodegenerative diseases that are characterized by the presence of progressive
cerebellar ataxia.
A retrospective cross-sectional study was conducted. 76 patients were
evaluated ranging from six to 63 years of age (mean age of 41.2) and underwent the
following procedures: case history, ENT and vestibular evaluations.
Clinically, the patients commonly had symptoms of gait disturbances (67.1%),
dizziness (47.3%), dysarthria (46%) and dysphagia (36.8%). In vestibular testing,
alterations were predominantly evident under caloric testing (77.6%), dismetric
saccades (50.0%), rotational chair testing (46%) and testing for gaze nystagmus
(43.4%). The presence of alterations occurred under examination in 85.5% of these
patients, 63.1% occurred in subjects with dominant ataxia and 22.4% occurred in
subjects with Friedreich's ataxia and 14.5% examination showed normal. The majority
occurring in those with central vestibular dysfunction, 68.4% of the examinations.
The most evident neurotological symptoms were gait disturbances,
dizziness, dysarthria and dysphagia. Alterations in vestibular examinations occurred in
85.5% of patients, mostly in the caloric test, with a predominance of deficient central
vestibular system dysfunction. This underscores the importance of the contribution of
topodiagnostic labyrinthine evaluations for neurodegenerative diseases since, in most
cases, the initial symptoms are neurotological, and these evaluations should also be
included in the choice of procedures to be performed in clinical and therapeutic
monitoring.
