Transition to non-active secondary progressive multiple sclerosis (SPMS) after up to 9.5 years of natalizumab treatment in the real-world TYSABRI Observational Program (TOP)

Explore transition to non-active SPMS in patients with relapsing-remitting multiple sclerosis treated with natalizumab in TOP.
In TOP, long-term natalizumab treatment was associated with low rates of disability worsening. This analysis explored disability progression that was neither preceded nor followed by relapse, suggestive of non-active disease.
Of the 6295 patients enrolled as of Nov 2018, those with baseline and ≥3 additional Expanded Disability Status Scale (EDSS) measurements were included (N=3871). EDSS scores were rebaselined after the last reported relapse to capture relapse-free progression events. Kaplan-Meier-estimated cumulative probabilities of transitioning to non-active SPMS were assessed with 4 exploratory definitions requiring EDSS score increase of 1.0 point from a score of ≤5.5 or 0.5 point from a score of ≥6.0 occurring in the absence of relapse, confirmed ≥3 months later, and resulted in either: 1) EDSS score ≥4.0 and minimum pyramidal functional system score ≥2, 2) EDSS score ≥4.0, 3) EDSS score ≥3.0, or 4) no EDSS threshold.
Fewer than 20% of patients met any of the non-active SPMS definitions. Estimated cumulative probabilities of meeting a non-active SPMS definition over 9.5 years ranged from 21.4% to 27.7%. Mean time from study initiation to definition ranged from 3.4 to 3.6 years. Mean EDSS scores at definition ranged from 5.0 to 5.8. Patients defined as non-active SPMS appeared older at study baseline and with longer disease duration, higher EDSS scores, fewer relapses in the year prior, and more prior disease-modifying therapies than non-SPMS patients. 
In TOP, data-driven definitions of relapse-free SPMS were explored. Consistent with clinical expectations, patients defined as non-active SPMS had more advanced disease characteristics at baseline. Across all definitions, rates of non-active SPMS ranged from 11.8% to 16.2%. Though limited by the lack of a comparator arm, these results support the long-term real-world effectiveness of natalizumab.
