Cryptococcal meningitis complicated by cerebellar ischemic strokes and obstructive hydrocephalus in a patient with multiple sclerosis on fingolimod

To describe a unique presentation of Cryptococcal meningitis in a patient with multiple sclerosis treated with fingolimod which was complicated by granulomatous perivascular inflammation resulting in bilateral cerebellar strokes and obstructive hydrocephalus.
It is recognized that immunosuppression with fingolimod in patients with multiple sclerosis does cause a predisposition for opportunistic infections including Cryptococcal meningitis. There have been reported cases of ischemic strokes associated with Cryptococcal meningitis and of leptomeningeal inflammation leading to obstructive hydrocephalus. This is a case of biopsy-proven Cryptococcal meningitis with vascular involvement of suppurative and granulomatous inflammation and complicated by bilateral cerebellar ischemic strokes and obstructive hydrocephalus.

Cryptococcal meningitis complicated by ischemic stroke appears to be a rarely reported phenomenon. This case with extensive cerebellar involvement and vascular involvement of inflammation demonstrated on biopsy is not previously described in the literature.
