Novel Pathogenic Variant in the SCN10A Gene Associated with Autosomal Dominant Small Fiber Neuropathy.

To describe a novel pathogenic variant in the SCN10A gene resulting in a loss-of-function effect associated with autosomal dominant small fiber neuropathy. 
No loss-of-function mutations have been described thus far in SCN10A-related disease.
Case report of two siblings seen at University of Kansas Medical Center (KUMC) Neuromuscular Clinics for seemingly cryptogenic neuropathy.
Patient 1 is a 67-year-old woman who had presumed CIDP previously treated with IVIg and a concomitant progressive small fiber neuropathy. On exam, she had stable distal weakness (finger abductors 4+/5, toe flexors 4/5), sensory deficit to pinprick to her mid-legs and finger tips, and trace patellar reflexes. Her genetic testing revealed a variant of unknown significance in the SCN10A gene [c4164G>A (p.Trp1388)]. Patient 2 is a 70-year-old man, the brother of patient 1, with a history of myeloma treated with lenalidomide and bortezomib one year prior, who presented to our clinic for a 13-year-long history of numbness from the knees down, dry eyes, orthostatic intolerance and mild sensory ataxia. Exam showed distal lower extremity sensory gradient. His EMG/nerve conduction study moderate sensorimotor axonal peripheral neuropathy. Genetic testing for patient 2 revealed the identical variant which would introduce a premature transitional stop codon resulting in an absent or disrupted protein i.e. loss-of-function mutation.
We describe 2 cases of a brother-sister duo with neuropathy likely caused by a novel variant in the SCN10A gene. Gain-of-function mutations in the SCN10a gene have been described by Faber et al. to cause Familial Episodic Pain Syndrome-2, but no loss-of-function mutations have been described thus far in SCN10A-related disease. Additional case reports, and in vitro mouse models would be beneficial in clarifying the phenotypic spectrum.
