Effects of Disease Modifying Therapy (DMT) Exposure on infant birth weight and birth length: Interim Data from Prospective PREG-MS Cohort.

To evaluate the effects of early pregnancy maternal DMT exposure on infant birth weight and length in a prospective, real-world multiple sclerosis (MS) pregnancy registry.
MS is a common neurologic disease affecting young women of childbearing age. DMT exposures in pregnant MS patients are common and associated risks are not well understood. There is paucity of real-world, high quality prospective data on a wide range of maternal and pediatric outcomes related to DMT-exposed pregnancies.

PREG-MS is the New England MS Pregnancy Prospective Cohort Study in the United States which follows women with MS in active conception attempts (ACA), and any stage of conception to three years postpartum. Participants’ MS course, DMTs, pregnancy, and their children’s development is collected through structured phone interviews and validated with medical records. We compared early neonatal outcomes between DMT exposed versus unexposed groups using simple linear regression adjusting for multiple disease-related variables, gestational age, and intrapartum complications.

To date, we received 177 referrals, of which 111 women are enrolled in the study. 80 (72%) subjects have delivered, 24 (22%) are currently pregnant and 16 (14%) are in ACA. 20 women are waiting to start ACA, and 30 have withdrawn consent or were lost to follow-up. 13 subjects miscarried and temporarily stopped study activities. 

7 (9%) subjects had preterm labor (32.30 + 4.48 weeks). 37 (46%) had pregnancy DMT exposure and 43 (54%) had no exposure. All pregnancies resulted in a live birth. Adjusted analysis suggested birth length (p=0.047), but not birth weight (p=0.184) differences between DMT exposed and unexposed babies with shorter length in exposed pregnancies.
PREG-MS interim adjusted analysis suggests possible effect of any DMT exposure in early pregnancy on the birth length, but not the birth weight of the infants. These preliminary results will need to be verified through ongoing prospective data collection.
