Acute Treatment Management Gaps in People With Migraine: Results of the CaMEO Study

To evaluate the acute treatment management gaps in people with migraine from the Chronic Migraine Epidemiology and Outcomes (CaMEO) Study.
Despite the wide range of medications available for the acute treatment of migraine attacks, many people with migraine report acute treatment management gaps. Many acute treatment options for migraine are associated with poor tolerability or limited effectiveness, or are contraindicated in patients with comorbidities.
Eligible respondents who met migraine International Classification of Headache Disorders, 3rd edition (ICHD-3) criteria completed the CaMEO Web-based survey and Comorbidities/Endophenotypes survey module. Six acute treatment management gap domains were defined and evaluated: (1) moderate or severe headache-related disability (Migraine Disability Assessment Scale [MIDAS] score ≥11); (2) lack of current acute treatment optimization (≥2 negative responses to the Migraine Treatment Optimization Questionnaire [mTOQ-5], with dichotomous scoring); (3) opioid or barbiturate use ≥4 days/month or acute medication overuse (by ICHD-3 definitions); (4) self-reported history of cardiovascular events (indicating possible contraindication to triptan use); (5) emergency department or urgent care use for headache within preceding 6 months; and (6) history of gastrointestinal events (indicating possible relative contraindication to NSAID use).
Data were provided by 12,810 respondents (11,699 with episodic migraine [EM], 1,111 with chronic migraine [CM]); 7,936 (62.0%) had ≥1 acute treatment management gap (58.8% of those with EM [6,881/11,699] and 95.0% [1,055/1,111] with CM). Among all respondents, 38.4% exhibited moderate or severe headache-related disability, 35.4% had non-optimized acute treatment, 18.6% reported medication overuse or barbiturate/opioid use ≥4 days/month, 6.2% reported a history of cardiovascular events, 4.8% reported emergency department or urgent care use for headache within the preceding 6 months, and 14.4% reported a history of gastrointestinal events.
Acute treatment management gaps are apparent among people with migraine and were greater for CM than EM, underscoring the needs for additional acute treatment options.
