Safety of Pregnancy After Cerebral Venous Thrombosis: Results From a National Comprehensive Hospital

To analyze the clinical characteristics and outcomes of female CVT patients of childbearing age, and to evaluate the risks associated to subsequent pregnancies.

Few studies have been published on female cerebral venous thrombosis (CVT) patients of childbearing age in China, and the safety of subsequent pregnancies in these patients is unclear.

We retrospectively analyzed the clinical data of women diagnosed with CVT in our Hospital between January 2009 and January 2019, and obtained long-term follow-up data to evaluate the recurrence rate of CVT or non-cerebral venous thrombosis, in particular during subsequent pregnancies.

Seventy-two patients (mean age at CVT onset 29.4 ± 7.8 years) were enrolled. The mean follow-up time was 63.1 ± 31.4 months, and 94.8% of the patients had a good outcome. During follow-up, there were 3 cases of CVT recurrence (including one during subsequent pregnancy) and 1 case of non-cerebral venous thrombotic recurrence. Among the 3 patients with CVT recurrence, one had paroxysmal nocturnal hemoglobinuria and two had antiphospholipid syndrome. Among the 17 pregnancies in 13 women during follow-up, 1 was in progress, 9 gave birth to healthy children, and 7 were terminated, including 3 spontaneous abortions: One patient with Behcet's disease had one, and one with APS had two.

Chinese female patients with a past history of CVT have a low risk of CVT recurrence during subsequent pregnancies. Patients with thrombophilia have higher risk of recurrence and spontaneous abortion. Antithrombotic prophylaxis and etiological treatment are recommended for these patients during pregnancy.
