Factors Associated with Depression after Hemorrhagic Stroke

To determine factors associated with depression after hemorrhagic stroke.
Depression may affect recovery and functional outcome after hemorrhagic stroke.
We prospectively identified patients with non-traumatic hemorrhagic stroke between January 2015 and February 2021 and telephonically performed the Neuro-QoL depression inventory at 3- and 12-months post-stroke. We examined the relationship between depression (T-score >50) and both history and illness severity. We also compared depression at 3-months and 12-months.
71 patients completed the 3-month call; 17 (24%) had depression. Patients with depression at 3-months were more likely to have history of depression (18% vs. 2%, p=0.014), SSRI use prior to admission (24% vs. 4%, p=0.01) and be unemployed (69% vs. 39%, p=0.047). On multivariate analysis of history of depression/SSRI use and employment, history of depression/SSRI use remained an independent predictor of depression 3-months post-stroke (p=0.018, OR 9 (95% CI 1.47-55.35)). 32 patients (45%) completed the 12-month call; 6 (23%) had depression. There was no agreement between depression at the two time points (κ=0.179); 22 patients (69%) did not have depression, 4 (13%) were only depressed at 3-months, 4 (13%) were only depressed at 12-months and 2 (6%) were depressed at both time points. Patients with depression at 12-months were more likely to have history of smoking (83% vs. 28%, p=0.022), premorbid mRS score >0 (67% vs. 15%, p=0.009), and higher APACHE II score at admission (median 16 (IQR 10.75-18.75) vs. 9 (IQR 6.75-12.5), p=0.043). On multivariate analysis, history of smoking and premorbid mRS score >0 remained significant predictors of depression 12-months post-stroke (p=0.032, OR 12.77 (95% CI 1.25-130.25) and p=0.016, OR 16.38 (95% CI 1.69-158.75), respectively)).
Presence of depression, and factors associated with depression, at 3-months and 12-months after hemorrhagic stroke differ. Further investigation is needed to tailor interventions to prevent and treat short- and long-term depression.
