A Novel, Urban and Rapid Interhospital Transfer System for the Neuroemergent Patient: A Subgroup Analysis of Intracerebral Hemorrhage

To describe the impact of the Neuroemergency Transfer Program (NTP) - - on ICH management and patient outcomes.
Prior work demonstrates, timely access to neuroemergent care in America is a significant public health concern, lacking in both rural and urban settings. 
A retrospective analysis of prospectively collected data was performed from 2008-2018 at Rush University in Chicago, IL. Patients ≥ 18 years old transferred via the NTP with ICH were included. 
2294 patients with ICH were identified with a male predominance of 53.6%. Mean patient age of 61.8 years (range 18-99) while 39.9% of patients were Black, 31.4% were Caucasian and 3.2 % were Asian. From 2008-2018 the annual count of ICH patient transfers and referral institutions increased from 184 to 282 and 79 to 97, respectively. Overall mean annual total transfer time and transfer distance were 118±3.7 minutes and 20.8±16.7 miles. 80.8% of patients were transferred from an ED vs. 14.2% from an ICU. Overall mean ICH score were 1.7. Mean ICU duration was 8.5±7.4 days with 36.0% of patients requiring a procedural intervention. Ultimately, 1296 (56.4%) patients were discharged home or to inpatient or outpatient rehabilitation. In total, 432 (18.6%) patients expired during the index admission from all causes, annual mortality rate varied without significance between 12.5-20.9% (p>0.05).
We report 11 years’ experience of ICH transfers via the NTP.  NTP facilitates expeditious transport and provide expert assessment and treatment upon arrival - all with the singular goal of improving patient outcomes.
