Correlations of the burden of lacunar infarcts with ischemic stroke recurrence and intracranial atherosclerosis burden in patients under anti-coagulation for non-valvular atrial fibrillation after acute ischemic stroke

To evaluate the effect of cerebral small vessel disease (SVD) on ischemic stroke recurrence in patients under anti-coagulation for non-valvular atrial fibrillation (NVAF) after acute ischemic stroke (AIS)
We sometimes observe that some patients experience recurrent ischemic stroke as lacunar infarction under anti-coagulation for NVAF after AIS. We hypothesize that ischemic stroke recurrence positively correlates with burden of lacunar infarcts in patients under anti-coagulation for NVAF after AIS.

We conducted a retrospective observational study of 150 ischemic stroke patients with NVAF who visited Seoul St. Mary’s Hospital from March 2014 to May 2018. We compared patients with recurrent ischemic stroke under anti-coagulation for NVAF after AIS and those without it by analyzing their stroke risk factors, atherosclerosis burden in CT or MR angiographic images and cerebral SVD burden in brain MRI. 

From one hundred and fifty patients from the registry composed of ischemic stroke patients with NVAF in Seoul St. Mary’s Hospital. Thirty patients showed the recurrence of ischemic stroke under anti-coagulation for NVAF after AIS. One hundred and three patients showed no recurrence of ischemic stroke. Remaining seventeen patients were excluded from analysis because some of their clinical and radiological information were missing. The burden of lacunar infarcts positively correlated with ischemic stroke recurrence in patients under anti-coagulation for NVAF after AIS. Interestingly, we found out that the burdens of cerebral SVD showed significant positive correlations with intracranial atherosclerosis (ICAS) burden in this study.

The positive correlation between the burden of lacunar infarcts and ischemic stroke recurrence in patients under anti-coagulation for NVAF after AIS implicates that these two stroke etiologies should be treated with different strategies for secondary prevention. On the contrary, the significant positive correlation between the burden of lacunar infarcts and ICAS burden suggests that lacunar infarcts and ICAS could be prevented with the same strategy.
