Serum Uric Acid as a Risk Factor in Acute Ischemic Non-Embolic Stroke

The study was conducted to determine the association between Serum Uric Acid (SUA) and
acute ischemic non-embolic stroke and to assess its risk factor potential.
Stroke is particularly important due to its high risk of recurrence. Many studies are still being conducted on its etiology and its other associated factors. One such factor, which has been the subject of controversy from long ago, is the blood level of uric acid. Preliminary studies indicated the role of serum level of uric acid in patients with myocardial infarction. The fact that thrombotic stroke and myocardial infarction are similar in many risk factors and pathophysiology, have resulted in considerable attention paid recently to the role of uric acid in patients with stroke.
100 subjects were included in the study. This study included two groups (cases and controls) with 50 subjects in each group. A detailed history and examination was carried out in each patient. Estimation of serum uric acid was carried out in patients reporting to emergency department within 24 hours of onset of symptoms of acute ischemic stroke. All cases underwent CT head and cases of hemorrhage were excluded from the study. MRI brain was done for confirming diagnosis if CT head was equivocal. Carotid Doppler, Electrocardiography, Echocardiography were done to rule out cases of embolism. The results of investigations performed on cases was compared and statistically analyzed with controls.
We found that mean serum uric acid levels were significantly higher among cases than
controls (6.23±1.16mg/dl, 5.14±0.95 mg/dl, P<0.001).

Serum uric acid levels were significantly higher among cases than controls, hence serum uric acid can be used as a marker for increased risk. Trial of serum uric acid lowering drugs in stroke patients as well as in those at increased risk of stroke is worth considering.
