Prevalence of Cardiovascular Diseases and Vascular Risk Factors in Patients with History of Epilepsy/Seizures

To describe the prevalence of common cardiovascular diseases and risk factors in patients with history of epilepsy/seizures (E/S).

Increased risk of new-onset seizures among stroke-free atrial fibrillation (AF) patients is well reported. Some reports also suggested a higher risk of AF and myocardial infarction in patients with epilepsy.

This study was conducted at King Abdullah University Hospital-Northern Jordan. All adults (> 18 years) with history of E/S between 2012 and 2018 were included. We calculated the prevalence of cardiovascular diseases and common vascular risk factors in this cohort and compared it to an age- and gender-matched cohort of patients who had no history of E/S (NON-E/S) but visited the neurology clinic during the same period.

A total of 937 patients were included, mean age 47 ± 19.7 years (males 53.1%). 726 (77.5%) patients had history of E/S. Comparing E/S with NON-E/S patients, the prevalence of ischemic heart disease was 12.7% versus 9.5% (P= 0.21); AF was 5 % versus 0.9 % (P= 0.009); congestive heart failure was 3.9% versus 1.4% (P= 0.08); and ischemic stroke was 21.3% versus 9.5% (P< 0.001).

There was no significant difference between the E/S and NON-E/S patients in the prevalence of diabetes, hypertension, hyperlipidemia and smoking. Obesity (BMI>30) was higher in NON-ES patients (41.5% vs 24.4% (P= 0.009)).

This study suggests that some cardiovascular diseases such as AF and ischemic stroke are more prevalent in patients with epilepsy, emphasizing the importance of screening and vigilance for such conditions in these patients.
