Acute Treatment with Oral Rimegepant 75mg Reduces Migraine-Related Disability: Results from a One Year, Open-Label Safety Study (BHV3000-201)

To assess the effects of rimegepant 75 mg on migraine-related disability over 52 weeks of use as an acute treatment for migraine.
Numerous epidemiologic studies in representative populations have shown that migraine is among the most disabling neurologic conditions in the world, with the burden falling most heavily on adults in their most productive years (age 25–55).

BHV3000-201 was an ongoing, multicenter, long-term, open-label safety study of rimegepant 75mg. Eligible subjects included adults with ≥1 year history of migraine (ICHD-3 beta) instructed to treat migraine attacks of any pain intensity with rimegepant 75 mg as needed up to once daily for up to 52 weeks. Disability was assessed at baseline and Weeks 12, 24, 36, and 52 via the Migraine Disability Assessment (MIDAS), a validated 5-item instrument that queries migraine-related absenteeism and lost productivity at work, school, and home, as well as missed family, social, and leisure activities. Mean changes from baseline in total scores were analyzed post-hoc with paired t-tests, with decreasing scores signifying benefit.

Sample sizes were n=1793, 1645, 1217, 1090, and 980 at baseline, weeks 12, 24, 36, and 52, respectively. Baseline mean total MIDAS score was 33.9+0.8 (SE) corresponding to the grade of severely disabled. Mean [95% confidence interval] change from baseline in MIDAS total score was −12.5 [−13.96,−11.12]; −14.2 [−15.80,−12.56]; −14.8 [−16.61,−13.04]; and −14.8 [−16.64,−12.90] at weeks 12, 24, 36 and 52, respectively. Improvements in MIDAS were clinically relevant and statistically significant at all time points (p<.0001).
Acute treatment with rimegepant 75 mg confers significant improvement in migraine-associated disability, transitioning patients from severe to moderate disability over time. Observed changes exceeded the minimum clinically important difference (5 disability days/3 months) by more than 2-fold at all time points. These benefits would favorably impact healthcare costs, workplace productivity, and patient well-being.
