Sudden Unexpected Death in Epilepsy (SUDEP), the SUDEP-7 inventory and other clinical risk factors

To assess surrogate markers for Sudden Unexpected Death in Epilepsy (SUDEP) in patients who died from SUDEP and refractory epilepsy patients using the revised SUDEP-7 inventory and subset risk factors such as number of generalized tonic clonic seizures (GTCs) and epilepsy duration. 
SUDEP is the leading cause of death in epilepsy patients. Multiple studies looking at potential risk factors including the SUDEP-7 inventory for risk stratification and counseling have revealed conflicting evidence. The validation of this inventory is important before routinely using it in research and in clinical practice.

 We compared the mean SUDEP-7 inventory scores, duration of epilepsy and presence of three or more generalized tonic clonic seizures (GTCs) per year in 11 cases died from SUDEP and 80 living patients with intractable epilepsy. The difference between the case and control groups was analyzed using the t-test in the first two parameters and Fisher’s exact test in the last parameter.
We analyzed SUDEP-7 inventory score in 11 SUDEP cases and 80 intractable epilepsy revealing a score of 5.81 ± 2.44 (range 2-10) in SUDEP cases, and 3.56 ± 1.94 (range 0-10) in refractory epilepsy group (p = 0.001). Nine of 11 SUDEP cases and 40 of 80 refractory group had 3 or more GTCs in the last year of follow up (p= 0.04). 

The mean duration of epilepsy was 26.18 ± 20.06 (range =5-66) in SUDEP cases, and 20.35 ± 12.83 (range 1-53) in refractory group (p = 0.19).
Contrary to recent reports, the SUDEP-7 inventory had a good predictive value for SUDEP risk with higher scores seen in SUDEP cases as compared to refractory epilepsy group with significant higher incidence of frequent GTCS.  Continued assessment of risk factors and markers for SUDEP is needed to provide opportunities for prevention and counseling.
