Anxiety symptoms in youth with and without tic disorders

To evaluate and compare anxiety in youth with and without tic disorders.
Chronic tic disorders (CTDs) affect 1-3% of youth; approximately half of these individuals have anxiety. It is not known if anxiety symptoms in CTDs are the same as anxiety symptoms in the general population. Understanding the anxiety phenotype in CTDs can inform future treatment trials.  
This is a secondary analysis of data from a cross-sectional study of the impact of tic disorders. Caregiver-child dyads were enrolled for a single study visit. Anxiety symptom severity was measured using the self-reported Multidimensional Anxiety Scale for Children (MASC). We compared MASC scores in youth with tic disorders and community controls. We also compared MASC scores in our tic sample to archival data of baseline MASC scores from the Child/Adolescent Anxiety Multimodal Study (CAMS), the largest randomized controlled intervention trial in childhood anxiety. CAMS data were used to represent anxiety disorders in the general pediatric population. 
The MASC was completed by 176 youth with tic disorders, 93 controls, and 488 CAMS participants. Ages were comparable in each group. There was a higher proportion of males in the tic group compared to the other groups. MASC total scores were higher in the tic group (mean 53.0, SD 11.2) than the controls (mean 45.4, SD 10.2; p<0.0001). MASC total scores were similar in the tic (median 52.0, IQR 44.0-60.8) and CAMS (median 51.0, IQR 42.0-60.0; p=0.13) groups. Separation anxiety severity was higher in the tic (median 59.0, IQR 51.3-70.0) than in the CAMS (median 49.0, IQR 43.0-55.5; p<0.0001) groups.

Anxiety symptom severity was higher in youth with tic disorders compared to community controls and similar to that of CAMS participants. Separation anxiety severity was higher in the tic group than in CAMS participants. This may have implications for targeting anxiety treatment in tic disorders.
