Quantitative MRI Brain Atrophy and IgG Subclass Profile: Cross Sectional Relationship In A Population of People with MS (PwMS)

To explore the relationship between volumetric brain MRI measurements and immunoglobulin sub-score values within a population of PwMS.
Multiple Sclerosis is thought to be an autoimmune disease with neurological manifestations that vary in degree, type and age of onset. Immunological markers include CSF oligoclonal bands (OCB) and IgG index. Genetic variants are major determinants of CSF antibody levels. Specific IgG subclass deficiencies (or combinations therein) might impact disease course, therapeutic efficacy and risk for infections. MS can also result in cortical and subcortical demyelination, as well as neuronal and axonal injury. A combination of immune profiling and quantitative brain atrophy measurements may lead to a stronger understanding of MS treatment selection and monitoring. 
Retrospective review of data collected through routine care in PwMS. Icometrix post imaging volumetric analysis was used with the following measurements being collected: Whole Brain Volume (WBV), White Matter Volume (WMV), Gray Matter Volume (GMV), Cortical Gray Matter Volume (cGMV), Lateral Ventricle Volume (LVV), Hippocampal Volume (HV), Left Hippocampal Volume (LHV), Right Hippocampal Volume, Thalamus Volume (TV), Left Thalamus Volume (LTV), Right Thalamus Volume (RTV), FLAIR Hyper-intensities Volume (FHV), T1 Hypo-intensities Volume (THV). Serum IgA, IgM, IgG and subclasses collected are as follows: IgG1, IgG2, IgG3, IgG4. 

Results: 276 PwMS (71% female, average age 53+/-10 years).  Significant correlations were determined by regression analysis (p<.05) identified potential relationships for: TV&IgG-4, LTV&IgG-4, RTV&IgG-4, FHV&IgG-3, FHV&IgG-4, THV&IgG-3, THV&IgG-4 . No relationships for IgM, IgA or other IgG subclass and MRI data were identified.

Conclusion: Quantified MRI measurements of disease impact in PwMS in areas reported to be important in MS also appear to be related to select IgG subclass profiles. Select immune profile might be related to disease impact and might impact therapy choice and monitoring. Further investigation into these relationships are necessary to determine the significance of these findings
