Diffusion Tensor Imaging of the Nucleus Basalis of Meynert Reveals Associations with Cognitive State in Patients with Multiple Sclerosis

To determine associations between microstructural properties of the nucleus basalis of Meynert (NBM) and cognitive outcomes in patients with multiple sclerosis (PwMS).
The NBM is comprised of a group of neurons in the basal forebrain with widespread connections to the cortical areas and subcortical structures. Previous studies have shown that the NBM is particularly vulnerable to neurodegeneration in Alzheimer’s disease and Parkinson’s disease. Microstructural damage, as reflected by increased diffusion tensor imaging (DTI)-derived  measures of diffusivity, have been shown to be related to cognitive impairment in these diseases. However, damage to the NBM in MS has been scarcely investigated, particularly with respect to impaired cognition.
73 PwMS patients underwent 3T MRI with a protocol that included a diffusion-weighted imaging acquisition. All PwMS underwent cognitive assessment with the Brief International Cognitive Assessment for MS (BICAMS), which incorporates the Symbol Digit Modalities Test (SDMT), Brief Visuospatial Memory Test-Revised (BVMT-R) and California Verbal Learning Test-2nd edition (CVLT-2). Cognitive assessments were z-scored based on normative data obtained in healthy controls. Standard DTI measures, including fractional anisotropy (FA), mean diffusivity (MD), axial diffusivity (AD) and radial diffusivity (RD) were calculated. A probabilistic map of the NBM was utilized to calculate DTI-derived measures. Pearson correlations were used to assess the relationship between BICAMS cognitive outcomes DTI assessments of the NBM.
SDMT scores were associated with NBM measures of FA (r=0.268, p=0.022), MD (r=-0.241, p=0.040), and RD (r=-0.249, p=-0.034). BVMT-R scores were associated with NBM measures of FA (r=0.245, p=0.036),  MD (r=-0.247, p=0.035), and RD (r=-0.25, p=0.029). No significant associations were found for CVLT-2. Associations between NBM and SDMT remained significant for MD and RD even after controlling for whole brain volume.

Our results highlight the role that damage to the NBM may potentially play in determining cognitive impairment in MS.
