Duration of Consecutive Migraine-Free Days Experienced After Treatment with Eptinezumab in Patients with Migraine

To evaluate the impact of eptinezumab vs placebo on duration of consecutive migraine-free days in patients with migraine. 
Extended periods of migraine freedom allow patients to recover from frequent assaults of migraine, which can afford quality of life increases for patients and may have favorable effects on disease pathophysiology. Eptinezumab, a CGRP monoclonal antibody administered intravenously, has demonstrated migraine preventive efficacy in patients with episodic migraine (EM) and chronic migraine (CM).
PROMISE-1 (EM; NCT02559895) and PROMISE-2 (CM; NCT02974153) were double-blind, randomized, placebo-controlled trials evaluating eptinezumab for the preventive treatment of migraine. A tertiary endpoint was the maximum duration of consecutive migraine-free days that started within 14 days of the first dose. These consecutive migraine-free days were then divided into four categories: >21 days, 15–21 days, 8–14 days, and ≤7 days. 
PROMISE-1 included 888 patients with EM (100mg, n=221; 300mg, n=222; placebo, n=222); PROMISE-2 included 1072 patients with CM (100mg, n=356; 300mg, n=350; placebo, n=366). In PROMISE-1, the maximum migraine-free duration starting within the first two weeks was >21 days in 29% (100mg, n=63) and 33% (300mg, n=73) vs 19% (placebo, n=41) of patients; it was ≤7 days in 21% (n=46) and 21% (n=46) vs 32% (n=71) of patients, respectively. In PROMISE-2, the maximum migraine-free duration starting within the first two weeks was >21 days in 21% (100mg, n=73) and 27% (300mg, n=94) vs 12% (placebo, n=42) of patients; it was ≤7 days in 41% (n=146) and 38% (n=132) vs 62% (n=226) of patients, respectively.
The duration of consecutive migraine-free days was greater with eptinezumab than with placebo in patients with migraine, starting within the first two weeks of treatment.
