Characterizing Long-Term Disability Progression Outcomes in NARCOMS Registry MS Participants Taking Dimethyl Fumarate

To determine the effects of dimethyl fumarate (DMF) on disability progression in persons with relapsing remitting multiple sclerosis (RRMS) treated for up to 5 years.
Data are lacking regarding the long-term effects of DMF on disability progression and patient-reported outcomes, including those related to physical and mental functioning. The NARCOMS registry is a voluntary self-report registry for people diagnosed with MS. Participants provide health-related information at enrollment and survey updates every 6 months.

This longitudinal study included US RRMS registry participants reporting DMF initiation between fall 2013 and spring 2018 with ≥1 year of follow-up. Disability progression was measured using the Patient Determined Disease Steps (PDDS), which has been shown in previous studies to be correlated with the Expanded Disability Status Scale. Time to 6-month confirmed PDDS progression (CPP, ≥1-point increase) was evaluated using Kaplan-Meier and Cox proportional hazards regression models adjusting for age, sex, income, and comorbidities. Participants were censored at last follow-up or DMF discontinuation, whichever came first.

Within the study period, 1164 RRMS participants initiated DMF; 553 had ≥1 year of follow-up data. Most participants were female (85.0%) and Caucasian (83.0%), while 54.2% had a bachelor’s degree or higher education. The mean (SD) age was 51.7 (9.6) years at DMF initiation and 37.7 (9.0) at diagnosis. The median (25%, 75%) PDDS level at initiation was 2 (0, 7). Median follow-up was 2.5 years. Of the 553 participants, 123 (22.2%) had CPP at median (25%, 75%) of 1.5 years (1, 2.5). 

NARCOMS participants who were treated with DMF for up to 5 years experienced low rates of disability progression. The NARCOMS registry provides an opportunity to longitudinally assess outcomes in DMF-treated MS patients. In addition to PDDS, other patient-reported outcomes will be presented.
