Automated EDSS scoring using a mobile App and Machine learning.

Quantitative assessments of cognition, sensory and motor functions are central to clinical decision-making for patients with Multiple Sclerosis. The Expanded Disability Status Scale (EDSS) is the current gold standard of metrics for MS.  A specially designed mobile app (the BeCare MS Link App) collects data as users complete different app-provided activities.  

Multiple Sclerosis (MS) is a chronic, disabling disease linked with progressive decline in neurological functioning.

Our research compared 35 subject's EDSS score which were preformed with a specialized mobile app derived EDSS score. The BeCare app was developed specifically to evaluate the symptomatology and neurologic function of patients with MS and to become a digital equivalent for the EDSS. The subjects were evaluated by MS neurologists at Weil Cornell Institute.

The data is provided to 4 different machine learning techniques with an independent EDSS score prediction generated from each.  These scores are then combined to calculate a single EDSS score along with a machine-generated estimate of the resulting score’s accuracy.The most accurate technique produced 19 scores that exactly matched the clinically-derived EDSS, 21 that produced scores within 1/2 point of the clinically-derived EDSS, and 32  that produced scores within within 1 point of the clinically-derived EDSS. The mean squared error was 0.857.   The range of clinician assigned EDSS scores were from 0 to 6.0

In conclusion the BeCare MS App can replicate the assessment of a MS patient by a clinically derived EDSS. As the EDSS is heavily weighted for ambulation and as assessments made by the App coupled with Machine Learning  include assessments of arm movement, cerebellar function, cranial nerve function and sensory function not assessed by the EDSS, this App may offer a more complete evaluation of disability in MS patients.
