A cross-sectional exploration of four CSF and plasma biomarkers across headache, multiple sclerosis, and neuromyelitis optica spectrum disorder

To conduct a preliminary exploration of the relationships between neurofilament light (NfL) with glial fibrillary acidic protein (GFAP), Tau, and Ubiquitin carboxy-terminal hydrolase L1 (UCH-L1) for patients with headache (HA), multiple sclerosis (MS), and neuromyelitis optica (NMO).

There is growing interest in identifying biomarkers to prognosticate and track treatment response in neurological diseases. The Quanterix SR-X system utilizes the Single Molecule Array (SIMOA) technology to measure multiple biomarkers in both cerebrospinal fluid (CSF) and plasma with an extraordinary level of sensitivity. 


Patients were identified from the Rocky Mountain MS Center in 1 of 3 groups: HA, MS and NMO with matched CSF and plasma samples (HA with opening pressures <20cm H2O). Clinical characteristics were obtained by chart review. Neuro 4 Plex A quantitative assays were conducted. Summary statistics including logarithmic transformations were generated along with correlations between NfL with GFAP, Tau, and UCH-L1, respectively. Only plasma concentrations are reported (pg/mL).

Total of 59 (HA=27, MS 24 and NMO= 8) subjects were included in the final sample. Mean age was 46.5(+/-11.2) years, 75% female, 78.9% Caucasian (10.5% African American and 8.8% Hispanic). Mean plasma concentrations were: 

NfLHA 5.51(+/-3.85); MS 11.52(+/-23.62); NMO 36.61(+/-75.40); 

GFAPHA 45.09(+/-38.70); MS 52.17(+/-32.68); NMO 537.54(+/-1195.12); 

TauHA 1.27(+/-6.77); MS 1.00(+/-0.50); NMO 2.24(+/-0.97); 

UCH-L1: HA  23.60(+/-40.54); MS 17.63(+/-11.40); NMO 76.41(+/-100.38). 

CSF concentrations showed a similar trend among disease states. In HA patients, NfL was strongly correlated to GFAP (r= 0.91, p<0.001), Tau (r= 0.77, p= <0.001), and UCH-L1 (r= 0.78, p= <0.001). In MS patients, the correlation between NfL and other biomarkers was moderate (GFAP (r= 0.42, p=0.038), Tau (r= 0.37, p= 0.068), and UCH-L1 (r= 0.46, p= 0.020). Less distinct trends were seen in NMO patients due to small sample size. 

Further exploration with larger sample sizes is needed to fully examine these relationships.
