Perspectives on Epilepsy-Related Fears Among Adult Patients Living With Epilepsy, Caregivers, and Healthcare Professionals

To examine epilepsy-related fears of adult patients living with epilepsy, their caregivers, and healthcare professionals (HCPs).
The online, 10-part STEP Survey (Seize the Truth of Epilepsy Perceptions) was conducted by Kantar Health on behalf of SK Life Science, Inc., in 2019.
Adult patients with epilepsy (n=400), caregivers (n=201), and HCPs (n=258) completed the STEP Survey. 
Of patients (mean age=40 years/mean epilepsy duration=16 years), 61% experienced ≥1 seizure with impaired awareness in the preceding year and 58% were treated with ≥3 antiepileptic drugs (AEDs). Irrespective of time since the most recent seizure, 83% strongly agreed they live in fear of another seizure and 39% reported that epilepsy produces constant fear. Worry about having a seizure in public was very or extremely disruptive to quality of life for 47% of patients. Epilepsy-related fear negatively influenced relationships, with 18% of patients afraid of entering long-term relationships.

Potentially affecting treatment decisions, 65% of patients somewhat or strongly agreed with not changing AEDs because of fear of seizures worsening. Additionally, 52% of patients were not at all or not very likely to ask their HCP about changing medications in the next 12 months, and 55% of this group feared deterioration in seizure control if AEDs changed. In contrast, 28% reported fear of epilepsy-related surgery.

Caregivers and HCPs perceive patients’ fear. Among caregivers, 58% somewhat or strongly agreed that patients do not switch physicians because of fear of starting over and losing gains. HCPs reported patients’ increased fear with an increasing number of AEDs tried, estimating 29%, 50%, and 51% of patients feeling afraid on the first, second, and third AED, respectively.
Fear pervades living with epilepsy and is recognized by patients, caregivers, and HCPs. Knowledge of the fear provides an opportunity to provide broader care and potentially reduce the impact of fear on treatment decisions.
