Identifying Gaps in Knowledge, Skill and Confidence Among MS Specialists to Facilitate Appropriate Evolutions in MS Care

To explore clinical practice gaps, their perceived causes and associated challenges as reported by healthcare providers specializing in MS (HPMS).
Evidence suggests that new data and the rapid pace of changes in MS care warrants regular assessment of the gaps in knowledge, skills and confidence levels of HPMS that may impact how these advances are implemented to inform and optimize decision-making.
Mixed-methods study consisting of qualitative interview and quantitative survey data including 330 neurologists and 135 advanced practice nurses in seven countries (US, UK, Canada, France, Spain, Germany, Italy). Thematic analysis of qualitative data, and ANOVA and chi-square analysis of quantitative data (stratified by country, years of experience, practice setting and MS certification status) were compared with previously published data on the subject.
Survey data indicates that 53% of nurses and 37% of neurologists reported low knowledge of the limitations of routine MRI techniques used in MS diagnosis and follow-up.  Biomarkers were reported to be infrequently used to assess progression/remission, particularly among HPMS with >20 years experience. Limited or no knowledge of available new MS treatments was reported by 32% of neurologists and 51% of nurses. Thirty-four percent of neurologists and 58% of advance practice nurses indicated only basic or no skills to integrate new treatments into practice. Qualitative interview data indicates that HPMS reported a perceived lack of skills to manage adverse events associated with B-cell targeted therapies.
This study documented self-reported knowledge gaps of HPMS with respect to the use and limitations of MRI techniques, fluid biomarkers (blood, cerebro-spinal fluid) and B&T-cell targeted therapies. The study documents the ongoing need for training opportunities, even at the practitioner level, to advance knowledge and decision-making skills to provide up-to-date MS patient care.
