Relationship between the severity of Obstructive Sleep Apnea syndrome and the intima media thickening of common carotid artery in patients with stroke

Determine the relationship between the severity of OSAS by the AHI with the IMT of the common carotid artery and the existence of atheromatous plaques in the carotid system.
The obstructive sleep apnea syndrome (OSAS) has been described as an independent risk factor either for stroke or atherosclerosis. It has been well documented a positive correlation between the intima-media thickening (IMT) of the common carotid artery in patients with OSAS and the severity of its disease by the apnea/hypopnea index (AHI). However, these studies showed this relationship in a population without history of cardiovascular or stroke disease.
For this study, there were included patients with a diagnosis of acute ischemic stroke during the period of January 2016 to September 2019 that were admitted in the Neurology Department of the Hospital “Dr. José Eleuterio González” in Monterrey, Nuevo León. The patients that were included had already a carotid ultrasound and a sleep polygraphy.
A total of 89 patients were studied with a mean age of 60.60 ± 13.92 and with a mean AHI of 18.67± 14.72. Of all patients, only 41 had OSAS (p=0.000) and  73.17% were male . A ratio differences was made between the groups of OSAS and the patients with carotid plaque in which 8 patients were found with a plaque greater than 50% (p=.154). Furthermore, 18 patients with OSAS were found with a carotid artery intima-media greater than 0.81 (p=.359). A Pearson’s correlation coefficient was made and there was no statistically significant difference neither between the AHI and the size of the plaque (p=0.154) nor the carotid artery intima-media (p=0.195. However, a correlation was found between the patients with carotid plaque and an intima-media thickening regardless the AHI.
There was not a correlation between the severity of OSAS and the IMT of the common carotid artery.
