Combined Capnocytophaga spp. and Fusobacterium Nucleatum Brain Abscess with Vibrio Cholera Infection: a Case Report

To report a rare case of combined Capnocytophaga and Fusobacterium nucleatum brain abscess with an overview of multiple previous Capnocytophaga brain abscesses and an examination of its risk factors.
Fusobacterium nucleatum is commonly found in human oral cavities whereas Capnocytophaga species is seen in various animal oropharyngeal tracts. This case was further complicated by an antecedent Vibrio cholera infection which emits a toxin called zonula occludens toxin (Zot) that effects the permeability of the blood brain barrier.  To the best of our knowledge, this is the very first case of this combined infection as described in the English literature.

A 32 year old male  presented for evaluation of a headache later diagnosed by biopsy confirmed combined Capnocytophaga and Fusobacterium nucleatum brain abscesses. We hypothesize that the patient received a cut a work from an animal source that allowed Capnocytophaga to enter the blood stream, transverse the heart via an atrial septal defect, cross the blood brain barrier that was impaired by Zot from the Vibrio cholera, and enter the brain alongside the Fusobacterium from the tooth cavity to cause this abscess.


Our case reviews a rare combined abscess with several complications and reviews the risk factors of multiple other Capnocytophaga and Fusobacterium nucleatum brain abscesses.
