Prognostic value of Serum MatrixMetalloproteinases-9 level in patients spontaneous Intracerebral Hemorrhage in North Indian Population

To determine the relationship between the serum MMP-9 level in patients with ICH and outcome measured by modified Rankin Scale (mRS) at three months. 
MMP-9 is an inflammatorymediator of proteolytic enzymes family which hasmultipleroles in extracellular matrix remodelling; BBB degradation and breakdown of vessel wall integrity may contribute to the pathophysiology of ICH. 
In this study, 250 patients with ICH recruited within 72 hours of onset from Neurology ward, Department of Neurology at All India Institute of Medical Sciences, New Delhi. After obtaining written informed consent from patients/proxy, 4ml of blood sample collected and analysed for MMP-9 level by ELISA method.  All subjects were telephonically followed by using mRS scaleat three months after the recruitment. 
Mean age of patients and controls were 54.9±12.8 and 55.5±12.8;35.2%were female. A total of 109 (43.6%) deaths were observed at three months.ICH cases had higher risk of mortality with elevated MMP9level at baseline compared to alive group (513.7±133.9 vs 470±132.942.9; p<0.004).ROC curve analysis; including age, ICH volume, IVH, GCS on admission, infratentorial bleed and MMP9 contributes 80% area under the curve for predicting mortality.
Increased level of Serum MMP-9 was found to be significant predictor of mortality at threeand six months but become non-significant in multivariable analysis in patients with ICH.
