Unusual Presentation of a Dystrophinopathy

To report a case that potentially expands the phenotypic spectrum of dystrophinopathies.

Dystrophinopathies can present as Duchenne’s, Becker’s or isolated cardiomyopathy; clinical phenotype predominantly being proximal weakness. 
A 12-year-old male presented to our institution in 1983 for exertion induced leg twitching that started at 9 years of age. He complained of fatigue, cramps, weakness in the legs followed by inwards turning of his feet, falls, and exercise induced darkening of urine. On examination, he was found to have bilateral high arches with hammertoes, right great toe extensor contracture deformity, calf muscle hypertrophy, asymmetric distal wasting of leg muscles, and tight heel chords. He had ankle dorsiflexion, eversion, and great toe extension weakness, and hyporeflexia. Gait was wide based, unstable with inverted and internally rotated feet. His brother needed a heart transplant at 39 years of age. CK was 2480. Electrodiagnostic study was suggestive of a neuropathic process but muscle biopsy was suggestive of a primary muscle disorder. He was given a presumptive clinical diagnosis of Charcot Marie Tooth (CMT) disease based on clinical exam findings (this was before availability of genetic testing). Due to healthcare access issues, he was not followed up for many years. During his recent clinic visit, he was reassessed, and a repeat electrodiagnostic study showed chronic myopathy with irritative features, echocardiogram showed low ejection fraction with significant left ventricular systolic dysfunction. Diagnosis under consideration was a muscular dystrophy of unknown type with CMT phenotype. Genetic testing was ordered and it revealed presence of pathological variant in DMD gene with deletions in exons 45-48 consistent with a dystrophinopathy.

This unusual clinical presentation of distal weakness and exam findings of CMT disease can potentially expand phenotypic spectrum of dystrophinopathies, but needs confirmation with additional cases.
