The Role of Insomnia in Migraine and Tension-type Headaches: A Population-Based Study

This study investigated the prevalence of insomnia in participants with migraine and tension-type headaches, and evaluated the impact of insomnia on the clinical presentation of migraine and tension-type headaches.
Insomnia and primary headaches, including migraine and tension-type headaches, are common disorders. Recent studies have reported a close association between insomnia and primary headaches. Although insomnia aggravates headache symptoms, information regarding the role of insomnia on migraine and tension-type headaches is limited.
We analyzed data from the Korean Headache-Sleep Study (KHSS) – a nation-wide, cross-sectional, population-based study on insomnia, depression, anxiety, headache type, and clinical characteristics of headache symptoms. Participants were surveyed according to a clustered random sampling method. Migraine and tension-type headache were classified based on the International Classification of Headache Disorders-3, beta. If the score on the insomnia severity index (ISI) was more than or equal to 10, the participant was classified as having insomnia.
A total of 2,695 participants were included in this study. The prevalence of insomnia was significantly higher in participants with migraines than in those with tension-type headaches (25.9% vs 13.2%, p<0.001) and those with non-headaches (25.9% vs 5.7%, p<0.001). Furthermore, the prevalence of insomnia was significantly higher in participants with tension-type headaches than in those with non-headaches (13.2% vs 5.7%, p<0.001). Migraine and tension-type headache participants with insomnia presented more severe headache intensity, anxiety, and depression compared to those without insomnia.
Insomnia was prevalent in participants with migraines and tension-type headaches. Furthermore, migraines and tension-type headaches accompanied with insomnia were associated with severe headache symptoms and psychiatric comorbidity. The evaluation of insomnia is essential for treatment in migraines and tension-type headaches.
